Transitions support in Hampshire and London

Epilepsy support in Hampshire and London

For 14-25 year olds with epilepsy, the Transition Support programme can provide additional help for those living in Hampshire and London.

The programme offers group and individual support in areas such as housing, independence, personal and social skills, and feelings of isolation.

Transitions Support Officers support young people, for example, at appointments, travel training, work experience, volunteering and employment.

I wanted to thank you for the support you are offering my patient. Her epilepsy is well controlled and the main aspect of her management is around her emotional and independence skills which you are supporting so well!
